Care Coordinator

Your PCN Care Coordinators are Claire Watson and Catherine Casey

'The Care Coordinator will work closely with the GP's and other primary care professionals to identify and manage a caseload of identified patients, making sure that appropriate personalised support is made available to them. They can help patients access support services to manage their own health and well-being such as, referring to social prescribers, health and well being coaches, and other health care professionals where appropriate.'

Our Care Coordinators help us to organise and run our wellbeing activities such as "Wild swimming" and "Singing for Wellbeing" courses

Our Cancer Care Coordinator will work with GP's and other primary care professionals to promote Early Cancer Diagnosis and Cancer Prevention. They raise awareness and provide advice, information and signposting to further support if needed
